Troup athletes must have physical


In order to participate in athletics during the 2018-2019 school year, all students must have a physical performed by a doctor.

Physicals are good for one year from the date they are performed. Students must have two forms, including the Pre-participation Physical Evaluation and Medical History and the Athletic Acknowledgement of forms completed and returned to the athletic trainer in order to participate.

First, the University Interscholastic League and Troup ISD require that students and/or parents be provided the following forms each year:

UIL Acknowledgement of Rules 

Concussion Acknowledgement form

Anabolic Steriod Use and Random Testing form

Sudden Cardiac Arrest form

TISD Drug Testing Authorization form

Second, the parent and/or guardian and student athlete must sign the Athletic Acknowledgement of forms.

Third, both the student athlete and a parent and/or guardian must complete the Pre-participation Physical Evaluation Medical History form.

Fourth, take the completed and signed Athletic Acknowledgement of forms and the Pre-participation Physical Exam form and Medical History on the athletic physical exam day at the high school, or have physical performed by a family physician.
